The State of Texas requires persons/companies who provide laser repair/services to register with the Texas Dept. of State Health Services (TxDSHS). The state provides a Certificate of Registration (Laser License Registration) for Laser Services. The University of Texas at Austin laser safety program will need a copy of this registration on file prior to any service or maintenance of regulated lasers inside the device. If you have not yet obtained the Certificate of Registration, please see the information below to begin the process.
Laser License Registrations on file with TxDSHS can be searched by name or organization at https://vo.ras.dshs.state.tx.us/datamart/mainMenu.do using the license search feature hyperlinked in the top paragraph under “Check License Status or Search for a License”.

Three main forms required to submit to TxDSHS.
- RC301-4 (Laser Services Application)
- RC226-1 (General legal business entity, identity and information)
- RC42-L (Applicant’s Laser Safety Officer’s credentials)
The Certificate of Registration for Laser Services is valid for any laser service work within the State of Texas, not just devices at our institution. There is a separate two-year application fee ($400) that will have to be submitted as well.
